*Please note waxing may have certain side effects such as redness, tenderness, hyperpigmentation, pimples, bumps, etc. Please, check each paragraph, acknowledging that you fully understand the following information:

I agree to adhere to all post-care safety, including, no strenuous activities, tight clothes, chemical peels, tanning or wet room services, swimming, spas, hot tubs, or other "heat induced activities" for 48 hours after waxing. This will ensure that the skin will not become irritated, inflamed or infected. For further details, please consult your esthetician. *

Please Be Advised to Avoid the Following:
- Use of a loofah or other abrasive exfoliates to the waxed area.
- Any topical prescriptions for 3 weeks prior to waxing and 10 days after. Products include: AHA's, BHA's, glycolic acid, salicylic acid and retinol derivatives
- Shaving in-between waxing appointments. Doing so will encourage ingrown hairs and will make hair more difficult to remove by waxing due to its coarse texture. (Hair removal around the perineum and labia is often coarse and grows in several different directions. It will take a consistent waxing regimen without shaving in between to remove all the hair in these areas).

Please be Advised to do the Following:
- it is suggested that you allow your hair to grow at least 9-14 days prior to your waxing appointment
- It is beneficial to the client to be on a consistent waxing schedule (every 4-6 weeks) when beginning a waxing routine. Hair will grow back less as the root is destroyed and you will be able to wait longer between sessions.

I have read the above information and if I have any concerns, I will address these with my esthetician. I give permission to my esthetician to perform the waxing
procedure we have discussed and will hold and her and her staff harmless from any liability that may result from this treatment. I have given an accurate account
of the questions asked above including all known allergies or prescription drugs or products I am currently ingesting or using topically. I understand my esthetician
will take every precaution to minimize or eliminate negative reactions as much as possible.
